The Impact Of Itensive Care Unit Services On Hospital Costs

INTRODUCTION: The most important factor in directing hospital managers’ studies to ensure and improve the economic performance is understanding units cost according to services. In our study; we aimed to determine the impact of 2010 Intensive Care Unit (ICU) services on the hospital costs in Haydarpasa Numune Training and Research Hospital.
METHODS: Our study is designed as retrospective. After getting the institution approval, face-to-face interview was performed with managers of hospital and services, and also salivary trustee, procurement processing files, accrue expense documents, branch staff, storage, revolving fund records were examined to collect information. In analysis of costs, fractional distribution method was applied.
RESULTS: ICU constitutes 3.43% of hospital bed count, though its cost has high ratio of 5.09% of total hospital cost. In the study, cost of intensive care unit was 6.112.864 TL, mean intensive care patient day cost was 711.625 TL. Hospital total cost was found to be 119.919.010 TL, excluding ICU was 113.806.146 TL and mean hospital patient day cost excluding ICU was 578.97 TL. The cost of ICU patient per day was higher than hospital patient per day cost in a ratio of 22.91%.
DISCUSSION AND CONCLUSION: The optimum cost should be maintained by developing data collection systems to provide qualified services together with continuous supervision performed to control costs in hospitals. This study will lead senior management to evaluate requests of ICU bed and staff number by their knowing that increasing intensive care bed number results in 22.91% higher costs than that of other units.

Yoğun Bakım Ünitesi Hizmetlerinin Hastane Maliyetlerine Etkisi

GİRİŞ ve AMAÇ: Hastane yöneticilerinin ekonomik performansın sağlanması ve yükseltilmesi için yapacakları çalışmalara yön veren en önemli unsur, birimlerin hizmetlerine göre maliyetlerini bilmeleridir. Araştırmamızda; Haydarpaşa Numune Eğitim ve Araştırma Hastanesi’nde 2010 yılı Yoğun Bakım Ünitesi (YBÜ) hizmetlerinin hastane maliyetlerine etkisini belirlemeyi amaçladık.
YÖNTEM ve GEREÇLER: Araştırmada retrospektrif yaklaşım benimsenmiştir. Kurum onayı alınarak hastane ve servis yöneticileri ile yüz yüze görüşmenin yanı sıra maaş mutemetliği, ihale işlem dosyaları, gider tahakkuk belgeleri, personel şubesi, depolar ve döner sermaye kayıtları incelenerek bilgi toplanmıştır. Maliyetlerin analizinde kademeli dağıtım yöntemi uygulanmıştır.
BULGULAR: YBÜ, Hastane yatak sayısının % 3,43’ünü oluştururken, maliyeti hastane toplam maliyetinin % 5,09’unu oluşturmaktadır. Araştırmada YBÜ maliyeti 6.112.864 TL. ve yoğun bakım hasta günü maliyeti ortalama 711,62 TL., Hastane toplam maliyeti 119.919.010 TL., YBÜ hariç 113.806.146 TL.ve hastane hasta günü maliyeti ortalama 578,97 TL. bulunmuştur. YBÜ hasta günü maliyeti hastane hasta günü maliyetinden ortalama %22,91 oranında daha fazladır.
TARTIŞMA ve SONUÇ: Hastanede maliyetleri kontrol altına almak için sürekli maliyet kontrolü yapılmalı ve veri toplama sistemleri geliştirilerek kaliteli hizmet sunumu ile birlikte optimum maliyet sağlanmalıdır. Araştırmanın, üst yönetimin yoğun bakım yatak sayısını artırmanın hastaneye maliyetinin diğer birimlere göre % 22,91 daha fazla olduğunu bilmesi, YBÜ yatak ve personel sayısının planlanmasında yol gösterici olacağı kanaatindeyiz.
