Mini Sling Operation in Stress Urinary Incontinence: The Assessment of Patients with Respect to Perioperative Complications, Operation Time, Hemorrhage and Pain

INTRODUCTION: Mini Sling operation is a new technique with a single incision, minimally invasive method of sling procedure that has short operation time and can perform under local anesthesia. The aim of this procedure is to decrease operation duration and complications related to other sling operations. This study aimed to explore perioperative complications, complaints and operation duration in patients underwent minisling procedure.
METHODS: A total of 25 women with stress urinary incontinence performed mini sling procedure. The demographic findings, gynecologic examination, stres test, Q-test, ped test, residual urine volume, operation duration and complications recorded. Urinary symptoms assessed by Urinary Distress Inventory 6 (UDI-6) and three points Likert scale applied to measure patient satisfaction.
RESULTS: The mean age of patients was 49,12±6.12 years. There were no peri-operative complications such as nerve-vessel injury, hemorrhage, hematoma, perforation, urinary retention, infection, mesh erosion. The mean operation duration of the procedure was 14,20±5,53 minute. The 32 % of the patients need analgesia within 24 hours of surgery. Then, there was no analgesic drug need unless colporrhaphy procedure added. The patient satisfaction was 96%.
DISCUSSION AND CONCLUSION: Mini sling operation is a procedure that has a single incision with decreased operation time, complications and analgesic drug need. These advantages of minimal invasive procedure make this operation attractive. However, studies with long term anatomical and functional results are needed.

Stres İnkontinansın Cerrahi Tedavisinde Mini Sling Yöntemi: Peroperatif Komplikasyon, Cerrahi Süresi, Kanama Miktarı ve Ağrı Açısından Değerlendirilmesi

GİRİŞ ve AMAÇ: Mini sling uygulaması tek insizyonla yapılan, lokal anesteziyle uygulanabilen, işlem olarak kısa süren minimal invaziv, yeni bir sling operasyonudur. Bu yöntemle askı ameliyatlarında oluşabilecek majör komplikasyonlardan kaçınma, operasyonu daha kısa süreye düşürme hedeflenmektedir. Bu çalışmada Mini sling tekniğinin peroperatif komplikasyonlara, şikayetler ve operasyon süresine etkisi araştırılmıştır.
YÖNTEM ve GEREÇLER: Stresle idrar kaçırma şikayetiyle başvuran 25 hastaya mini sling işlemi yapıldı. Hastalar intraoperatif ve erken dönem postoperatif, kanama, ağrı, yaşam kalitesi ve üriner retansiyon açısından değerlendirildi. Demografik özellikleri, jinekolojik muayene bulguları, stres test, Q-tip test, ped testi, residual idrar volümü, operasyon süresi ve komplikasyonları kaydedildi. Hastalara Urinary Distress Inventory (UDI-6) anketi ve hasta memnuniyeti ise üçlü Likert testi ile yapıldı.
BULGULAR: Hastaların ortalama yaşı 49,12±6.12 yıl idi. Peroperatif bir komplikasyonla karşılaşılmadı (sinir-damar yaralanması, hemoraji, hematom, perforasyon, idrar retansiyonu, infeksiyon, mesh erozyonu). Ortalama operasyon süresi 14,20±5,53 dakikadır. İlk 24 saatte analjezik ihtiyacı %32 ‘dir. Sonrasında ise kolporafi anterior gibi ek operasyon yapılmadığı sürece analjesik ihtiyacı yoktur. Hasta memnuniyeti %96 bulundu.
TARTIŞMA ve SONUÇ: Mini sling tekniğinin tek insizyonla ve lokal anestezi yapılabilmesi, peroperatif komplikasyonların olmaması, analjezik ihtiyacının ve operasyon süresinin az olması bu cerrahi prosedürü cazip hale getirmektedir. Ancak prosedürün uzun dönemli anatomik ve fonksiyonel sonuçlarıyla ilgili çalışmalara ihtiyaç vardır.
