INTRODUCTION: In this study, we aimed to evaluate clinical and radiological findings of 20 patients diagnosed with May-Thurner Syndrome in terms of radiological treatment methods.
METHODS: In the radiology department of our hospital; A total of 20 patients diagnosed with May-Thurner Syndrome radiologically were included in the study. Multidetector Computed Tomography (MDCT) in venography; In cases with more than 80% compression in the iliac vein; Conventional venography was performed to confirm the diagnosis and for endovascular treatment. cases; The degree of compression, clinical and radiological findings, radiological follow-up and treatment process were evaluated.
RESULTS: Two of the cases were asymptomatic and incidentally detected.5 cases had superficial-deep venous insufficiency. Acute deep vein thrombosis (DVT) was detected in one of the cases and chronic DVT in 9 cases. In 3 cases, superficial, deep venous insufficiency and accompanying chronic DVT findings were observed. Four of the cases underwent endovascular theraphy. Aftertreatment.Control MDCT venography, stents of all cases were patent.
DISCUSSION AND CONCLUSION: In the differential diagnosis of patients with venous insufficiency and /or recurrent DVT of left lower extremity,May-Thurner Syndrome must be kept in mind;it is necessary for the treatment and prevention of possible complications on time.In cases with significant compression findings, endovascular treatment can be used as an alternative to surgical treatment.
GİRİŞ ve AMAÇ: May-Thurner Sendromu tanılı 20 olgunun klinik ve radyolojik bulgularının sunulması, radyolojik tedavi yöntemleri ile sonuçları bakımından değerlendirilmesi.
YÖNTEM ve GEREÇLER: Hastanemiz radyoloji bölümünde; radyolojik olarak May-Thurner Sendromu tanısı alan toplam 20 olgu çalışmaya dahil edildi. Multidedektör Bilgisayarlı Tomografi (MDBT) venografide; iliak vende %80’in üzerinde bası tesbit edilen olgularda; tanının doğrulanması ve endovasküler tedavi amaçlı konvansiyonel venografi yapıldı. Olgular; bası derecesi, klinik ve radyolojik bulguları, radyolojik takip ve tedavi süreci açısından değerlendirildi.
BULGULAR: Olguların 2 tanesi asemptomatik olup insidental tesbit edildi. Olguların 5’inde yüzeyel-derin venöz yetmezlik,1’inde akut derin ven trombozu (DVT), 9’unda kronik DVT, 3’ünde yüzeyel, derin venöz yetmezlik ve eşlik eden kronik DVT bulguları mevcuttu. Olguların hepsine MDBT Venografi görüntüleme yapıldı. 4 olguya endovasküler tedavi uygulanmış olup kontrol MDBT görüntülerinde stentler patent görünümdeydi.
TARTIŞMA ve SONUÇ: Sol alt ekstremitede venöz yetmezlik bulguları ve/veya tekrarlayan DVT kliniği bulunan hastalarda; ayırıcı tanıda May-Thurner Sendromunun hatırlanması; uygun tedavinin yapılabilmesi ve olası komplikasyonların önlenebilmesi için gereklidir. Belirgin bası bulguları olan olgularda, endovasküler tedavi cerrahi tedaviye alternatif olarak kullanılabilir.
