Evaluation of Henoch-Schönlein Purport cases with edematous course

Henoch Schönlein purpura(HSP), is the most commonly seen vaskulites of childhood, primarily affects postcapillary venules of dermis, gastrointestinal tract, joint and kidney. It is mainly seen in children between 2 and 8 years with a male to famale ratio of 1,5. Systemic symptoms can be presented in %80 of patients and the most commonly seen symptom is purpuric rash. Arthritis, renal involvement and gastrointestinal disorders are the other manifestations. The incidence of soft tissue edema correlated with HSP is reported in %4-50 of cases in literature In this study, we reported 6 children whose ages are between 2,5 and 11 years, presented to our clinic with scalp,scrotal, perineal and ankle swelling. Our 1 patient was operated with testicular torsion. We showed that in the presence of soft tissue swelling, using prednizolon therapy can relieve the symptoms and discomfort. When we reported this edematous course of Henoch Schönlein purpura cases, we evaluated the disease again in the light of the literature.

Ödematöz Seyirli Henoch-Schönlein Purpuralı Olguların Değerlendirilmesi

Henoch Schonlein purpurası (HSP), birden çok sistemi etkileyebilen, öncelikle deri, gastrointestinal sistem, eklem ve böbreğin küçük çaplı kan damarlarını, özellikle de postkapiller venülleri tutan çocukluk çağının en sık görülen vaskülitidir. En sık 2-8 yaş arasında görülür. Erkek/kız oranı 1,5 dır. Sistemik bulgular hastaların %80 inde ortaya çıkar ve en sık bulgu purpurik deri döküntüsüdür. Diğer sık görülen bulgular artrit, böbrek tutulumu ve gastrointestinal problemlerdir. HSP vaskülitine bağlı yumuşak doku ödemi insidansı litaratürde %4-50 oranlarında bildirilmektedir. (1) Biz bu çalışmada yaşları 2,5 ile 11 arasında değişen scalp, testis, sırt, bel ve ayak bileği bölgelerinde ödemlerle gelen 6 çocuk hastamızı bildirdik. Bir hastamız testis torsiyonu ile opere oldu. Hastalarımıza doku ve skrotal ödem varlığında kullanılan prednizolonun semptomları rahatlatarak tedavideki yerine dikkat çektik. Özellikle, ödamatöz seyreden Henoch Schonlein olgularımızı sunarak, litaratür ışığında hastalığı tekrar gözden geçirdik.
